Kidneys, age, pregnancy, and other special situations

Gabapentin leaves the body through the kidneys as unchanged drug. Age, pregnancy, breastfeeding, and reduced kidney function change the clinical conversation. They do not create a website-generated dose.

Why kidney function sits at the center

Neurontin labeling states that gabapentin is eliminated by renal excretion as unchanged drug and is not appreciably metabolized. When kidneys filter less, gabapentin stays in the body longer and exposure rises. That is why adult labeling includes dosage adjustment tables based on creatinine clearance and extra guidance for hemodialysis.

Those tables are for clinicians. They require a recent, appropriate estimate of kidney function and knowledge of the indication and product. Publishing a copy of a dosing table here would invite people to plug in a lab number and treat the result as a prescription. We will not do that.

What readers can do is make sure the prescriber and pharmacist know about chronic kidney disease, a recent drop in kidney function, dialysis, or medicines that affect the kidney, and ask whether the gabapentin product and schedule were chosen with that in mind.

Older adults

Breastfeeding and pediatric use

Whether and how to use gabapentin during breastfeeding is a clinician-and-pharmacist question that depends on the product, the infant’s age and health, and the indication. Pediatric labeled use for immediate-release gabapentin is adjunctive therapy of partial-onset seizures in children 3 years and older, with weight-based regimens in labeling. Caregivers should not crush extended-release adult products to create a child dose.

Breathing disease and other medical context

People with COPD or other conditions that reduce lung function are specifically called out in FDA’s gabapentinoid warning. Sleep apnea, recent surgery, and concurrent opioids add further complexity. These are reasons for individualized monitoring, not for internet ranking of “safe lung conditions.”
