Sleep, mood, and nerve pain: connected, not interchangeable
Pain disrupts sleep; poor sleep amplifies pain. Gabapentin can cause sleepiness and carries a class warning about suicidal thoughts. None of that makes it a home sleep or antidepressant therapy.
Why nights get tangled with nerve pain
Neuropathic sensations often intensify at rest, when sheets touch the skin, or when there is nothing else competing for attention. Lost sleep then lowers pain tolerance the next day. That loop is real and is a fair topic for a clinic visit.
Treating only one strand—sleeping pills without a pain plan, or a gabapentinoid without sleep hygiene—can leave the loop intact. Ask what the team is targeting first.
Sedation is not the same as restorative sleep
Gabapentin labeling lists somnolence as a common effect. Feeling knocked out is not proof of healthy sleep architecture, and next-day unsteadiness can cause falls. Combining gabapentin with alcohol or other sedatives to “finally sleep” increases respiratory-depression risk highlighted by FDA.
Horizant is labeled for restless legs syndrome, a condition that wrecks sleep for some adults; that is a specific product and indication, not a license to use leftover generic gabapentin as a nightcap.
Mood monitoring is part of antiepileptic-drug safety
Antiepileptic drugs, including gabapentin, increase the risk of suicidal thoughts or behavior in class-wide analyses, for seizure uses and for other uses. New hopelessness, agitation, or talk of self-harm needs urgent clinical or crisis help—not a higher gabapentin dose taken to “settle nerves.”
Chronic pain and depression often coexist. That overlap is a reason to involve mental-health care, not a reason to assume gabapentin is an antidepressant. It is not labeled as one.
Practical night-time safety
If sedation is prominent, discuss night lights, bathroom fall risk, and whether driving to an early shift is still reasonable. Caregivers should know that extreme unresponsiveness is an emergency, especially if opioids are in the home.
